Tobacco cessation program: Quit For Life®
The Quit For Life Program is a free, voluntary tobacco cessation program available to all employees and their dependents (age 18 and older) through the American Cancer Society and Optum. This program creates a plan based on the participant’s lifestyle, preferences and tobacco use history to help you stop using tobacco products. Services offered under the Quit For Life Program include:
- Eight weeks of nicotine replacement therapy
- Counseling and support
- Five telephone counseling sessions
- Relapse counseling
- A six-month follow-up session
Report your tobacco use status at enrollment — avoid the fee
Your cost for medical, Supplemental Life Insurance and Spouse Life Insurance depends on the tobacco and/or smoke products (including vapor and e-cigarettes) used by you or any dependents you intend to cover. During your enrollment, you will be asked to complete the tobacco status questions.
You will avoid the fee/higher costs if your responses show you and/or your applicable dependents:
- Don’t use tobacco and/or smoke products, or
- Use tobacco and/or smoke products but enroll in the Quit For Life Program.
If you do not complete the tobacco status questions when you enroll, or if you use tobacco products and don’t intend to quit, you will be charged the $50 tobacco fee for medical, and tobacco user rates for Supplemental Life Insurance, and/or Spouse/Domestic Partner Life Insurance if you enroll in these benefits in 2020.
